Ex vivo and in vivo editing

Ex vivo means editing cells outside the body and putting them back; in vivo means editing them where they are.

简短解答

There are two ways to edit someone. You can take cells out, edit them in a facility, check them, and give them back — that is ex vivo, and it is how the approved therapy works. Or you can inject the editing machinery and let it find the cells inside the body — in vivo, which is far easier on the patient but cannot be inspected before it happens.

Ex vivo editing permits controlled conditions, quality control and release testing before administration, but is limited to accessible cell types and generally requires conditioning to enable engraftment. In vivo editing avoids cell manufacturing and conditioning but exposes the whole body to the editor, offers no opportunity to verify the product before it acts, and depends entirely on delivery tropism.

Two ways to give someone an edited genome Ex vivo — edit the cells outside the body collect cells edit in the lab check the edit clear space in the marrow infuse back Precise and checkable — but it is a months-long hospital procedure, and the conditioning step is itself harsh. In vivo — send the editor into the body one infusion carrier reaches the target organ editing happens inside you Far simpler for the patient — but you cannot inspect the cells first, and you cannot take it back.
Casgevy is ex vivo. Intellia's ATTR programme is in vivo. The difference shapes cost, access and risk more than the choice of editing tool does.

The trade-off in one table

Ex vivoIn vivo
Where editing happensIn a manufacturing facilityInside the patient
Can you check before giving?Yes — the product is tested and releasedNo
Patient burdenHigh — apheresis, conditioning chemotherapy, weeks in hospitalLow — an infusion
Cell typesBlood and immune cells, mostlyWhatever the delivery vehicle reaches
If something goes wrongThe product can be discarded before useIt cannot be withdrawn
Cost and complexityVery high — bespoke manufacturing per patientLower — a manufactured product

The last row is why in vivo is where most of the field is heading, and the row above it is why in vivo safety events are taken so seriously.

Edited cells are frozen and quality-checked before they go back into a patient — a step in vivo editing does not have.

The return infusion itself is quick; the months of preparation and recovery around it are the hard part.
